Yes. S316L Surgical Steel contains nickel, as almost all austenitic stainless steels do, because nickel is what makes that structure stable. That sounds like a problem and it usually isn't, because what affects skin is how much a metal releases rather than what it contains. Those are different measurements with different answers. Does surgical steel contain nickel? The honest answer first

Any brand telling you their surgical steel contains no nickel is either using a specialist nickel-free grade, which is rare and expensive, or they haven't checked.

Standard austenitic grades, including 316L, contain roughly 10 to 14 percent nickel. It's there for a structural reason: nickel stabilizes the austenitic crystal structure, and that structure is what gives the steel its corrosion resistance and its formability.

Take the nickel out and you don't get a safer version of the same metal. You get a different metal with different properties.

Presence and release aren't the same measurement

This is the distinction that resolves the whole question, and it's the one almost no product page explains.

Presence asks what elements are in the alloy. It's answered by composition.

Release asks how much of an element actually leaves the metal and enters skin during wear. It's answered by testing.

Only the second one has any bearing on her ears. Skin doesn't respond to what's listed in a specification. It responds to what actually reaches it.

And the gap between the two is large. Nickel bound tightly inside a stable metal structure can release almost nothing measurable. Nickel in an unstable alloy, or in a piece that's corroding, releases considerably more. Same element, entirely different outcomes.

That's why a stable steel containing 12 percent nickel can be a better choice for sensitive ears than a piece of jewelry containing far less nickel in a less stable form.

What holds the nickel in

Three things about S316L specifically.

Low carbon content. The L means low carbon, which reduces chromium carbide forming at grain boundaries. Those carbides create weak points where corrosion begins, and corrosion is what liberates metal ions.

Molybdenum. This is what separates 316 from the more common 304. It substantially improves resistance to pitting and to chlorides, which are present in perspiration.

A passive oxide layer. Chromium in the alloy forms a thin, stable, self-repairing oxide film on the surface. That layer is what stands between the alloy and the skin, and it re-forms if scratched.

The combined effect is a metal that sits in a warm, damp, mildly acidic environment without corroding much. Which is a description of a pierced ear, and also roughly what a surgical implant experiences.

How release gets measured

EN 1811 is the European standard, and it's a genuinely good test.

An item is immersed in artificial sweat at body temperature for a week, simulating prolonged skin contact. The solution is then analyzed for how much nickel has come out.

The European Union sets a limit on nickel release for items in prolonged skin contact, and a stricter one for items inserted into pierced ears. That distinction exists because a piercing channel is a more permeable environment than the outside of the skin.

A result of "not detected" means the nickel released fell below the analytical detection threshold of the test. It's a measurement rather than an adjective.

Worth knowing: the United States has no legal limit on nickel release from jewelry. The EU introduced one in 1994 and tightened it in 2004. Denmark restricted it in 1992, and nickel allergy among Danish children fell from 24.8 percent to 9.2 percent between 1985 and 1998. In the US, that decision sits with the person buying.

Why "nickel-free" is a weaker claim than it looks

Three reasons.

It's about composition rather than release, and release is what matters.

It's usually untested. Nothing requires substantiation before the phrase goes on a listing.

It often covers only part of the item. Posts and backs are frequently bought in from a different supplier than the earrings themselves, so a claim about an earring may not describe the components with the most skin contact.

That third one is the one nobody mentions. A brand can accurately say its earrings are nickel-free and be describing components that were never tested and possibly never examined.

Does this matter for a child?

More than for an adult, because of timing.

Nickel allergy affects around one in five people in North America. The American Academy of Pediatrics reported in 2020 that nickel allergic contact dermatitis in children has nearly quadrupled over three decades, with at least 1.1 million children in the United States already sensitized. A 2025 systematic review found people with piercings were nearly six times more likely to be nickel allergic.

Sensitization is cumulative and generally permanent. The immune system reacts once it's been exposed enough, over enough time, to learn the metal. Childhood is when that exposure starts, so the everyday material matters more than any single pair.

Reducing exposure early is straightforward. Managing a lifelong sensitivity isn't.

What a good answer sounds like

If you ask a seller about nickel and they say "our steel is nickel-free", treat that as a prompt for one more question rather than as an answer. Which grade, and was it tested?

A good answer sounds like this: it is 316L, which contains nickel as all austenitic grades do, it was tested under EN 1811 by a named laboratory, and the release result was below the detection threshold. That is longer and less reassuring-sounding, and it is the one you can actually check.

When I sent pieces to SGS I chose the ones with the largest surface area touching an ear, and I had the setting, the post and the butterfly back tested. Eight pairs, all of them. That scope is the part worth noticing, because backs are commonly bought in from a different supplier, which means many brands couldn't include them in a test even if they wanted to.

Nickel release: not detected under EN 1811. Tested to ASTM F2923 and CPSIA, with lead below a tenth of the United States legal limit. Report BTO 341803/1, and you can ask me for it.

Questions parents ask next

Is there a nickel-free surgical steel? Specialist nickel-free austenitic grades exist, usually using manganese and nitrogen in place of nickel. They're uncommon in jewelry and considerably more expensive.

If she's nickel allergic, can she wear S316L? Many people who are nickel sensitive wear it without difficulty, because release is very low. Sensitivity varies between individuals though, so a wear trial with a return policy behind it is the sensible way to find out.

Is titanium a better choice for nickel allergy? Implant-grade titanium contains no nickel at all, so it removes the question entirely. It's harder to work into fine settings and grades vary, so the name alone isn't enough there either.

Does the nickel come out faster as the earring ages? It can, if the surface corrodes or is damaged. That's precisely why corrosion resistance is the property worth caring about.

What about 304 stainless steel? Also contains nickel, with less corrosion resistance than 316 because it lacks molybdenum. For prolonged skin contact, 316L is the better specified choice.
