Is titanium better than surgical steel for earrings?

Titanium contains no nickel. Surgical steel contains nickel and releases almost none. Here is how the two compare for children's earrings.

On one measure titanium wins clearly: it contains no nickel at all, so the question of nickel release doesn't arise. On the measures that decide most purchases, the two are closer than that suggests, because well-specified S316L Surgical Steel releases almost nothing despite containing nickel. Both are good. Which is better depends on what you're solving for. Our Polished Surgical Steel collection uses the second.

The row that decides it for most people is the first one, and the second row is the reason it shouldn't automatically.

Why titanium's headline advantage is real

Titanium is genuinely inert. It forms an extremely stable oxide layer, it doesn't corrode in the conditions a pierced ear provides, and commercially pure grades contain no nickel whatsoever.

For someone with a confirmed, strong nickel allergy, that removes an entire category of uncertainty. There's nothing to release and nothing to test for.

It's also very light, roughly half the density of steel, which matters on a soft lobe over years.

Why it isn't automatically the answer

Three reasons, and the first is the big one.

"Titanium" alone is as uninformative as "stainless steel" alone. There are numerous grades. Commercially pure titanium comes in several designations. Ti-6Al-4V, a very common alloy, contains aluminum and vanadium, and the ELI variant is the one specified for implants. A listing saying only "titanium" hasn't told you which of these you have, exactly as "surgical steel" doesn't tell you 316L from 430.

Anodised colors. Titanium is often anodised to produce colors, which is a surface oxide effect rather than a coating. It's generally stable, and it does wear, and a worn anodised layer changes the surface inside the piercing.

It's harder to work. Titanium is more difficult to machine and to set stones into. In practice that means fewer fine settings, fewer designs, and a higher price for equivalent work. For a child who wants a particular look, that's a real constraint rather than a technicality.

The nickel question, in proportion

This is where the comparison usually gets decided badly.

S316L contains nickel. That fact, stated alone, sounds disqualifying next to a metal that contains none.

But presence and release are different measurements, and only release affects skin.

Nickel bound inside a stable austenitic structure with an intact chromium oxide film releases almost nothing. EN 1811 is the European test that measures it: a week in artificial sweat at body temperature, then analysis of what came out. A result below the detection threshold means the amount that left the metal was too small to measure.

So the practical comparison isn't "no nickel versus some nickel". It's "no nickel versus nickel that stays where it is". For most children, both produce the same outcome, which is nothing happening.

Where titanium retains a genuine edge is at the extreme: a child with a confirmed strong nickel sensitivity, established by patch testing. For her, removing the variable entirely is worth the trade-offs.

Where each one wins

Choose titanium if she has a confirmed nickel allergy from patch testing, if she has reacted to well-specified steel before, or if minimum possible weight matters for a particular reason.

Choose S316L Surgical Steel if you want a wider range of designs and finer stone settings, if you want a lower price for equivalent quality, or if you want a material with a long, well-documented history in both medical instruments and body jewelry.

Choose neither if the listing won't tell you the grade. That applies equally to both. An unspecified titanium earring and an unspecified steel one leave you in exactly the same position.

What actually matters more than the choice between them

Three things, all of which outrank titanium versus steel.

Whether the grade is named. A specified 316L beats an unspecified titanium every time, because you can look one up and not the other.

Whether the post and the back match the front. Backs are commonly bought in from a different supplier than the earrings. A brand can accurately describe an earring as titanium and have a back that came from somewhere else entirely. The post sits inside the piercing channel, where skin is thinner and more permeable than on the outside of the lobe.

Whether anything was tested, by whom, with what result. "Tested" isn't a result. A named laboratory and a stated outcome is.

Get those three right and the choice between two good materials matters much less than it seems.

Why this matters for a child specifically

Nickel allergy affects around one in five people in North America. The American Academy of Pediatrics reported in 2020 that nickel allergic contact dermatitis in children has nearly quadrupled over three decades, with at least 1.1 million children in the United States already sensitized. A 2025 systematic review found people with piercings were nearly six times more likely to be nickel allergic.

Sensitization is cumulative and generally permanent. The immune system reacts once it's been exposed enough, over enough time, to learn the metal. Childhood is when that exposure begins, which is why the everyday earring matters more than any single special pair.

That's the argument for caring about the material at all. It isn't an argument for titanium over steel specifically. It's an argument for knowing what she's wearing.

What we chose and why

purepixxi uses S316L Surgical Steel rather than titanium, and the reasoning is worth setting out plainly.

The testing answers the nickel question directly. Nickel release not detected under EN 1811 is a measured result on our actual product, rather than an inference from a composition table. It's a stronger form of evidence than "contains no nickel" is, because it describes behavior rather than ingredients.

It allows the settings we want. Stones set flush and level in fine settings, on a 4mm or 5mm stud sized for a young ear. Titanium would narrow that considerably.

It lets the standard run all the way down. The setting, the post and the butterfly back all carry the same S316L Surgical Steel core, from one manufacturer, which is what makes it possible to test all three components together.

And it keeps the price reachable. A material choice that puts the product out of range doesn't protect anybody's ears.

I turned down suppliers rather than accept specifications I couldn't verify, and this is the same principle applied to material selection. There's more available on the market in styles and options. Taking it would have meant giving up the ability to say exactly what every component is.

How to read a titanium listing

The same discipline that applies to steel applies here, and almost nobody uses it on titanium because the word sounds technical enough to be self-explanatory.

Look for a designation. Commercially pure titanium is graded, and implant alloys carry their own specifications. A listing with the word and no designation is describing a category rather than a material.

Ask whether it is anodized, and what color. Anodizing is an oxide effect rather than an applied coating, so it does not flake the way plating does. It does wear at points of friction, and a worn area looks different from the rest.

Ask about the post and the back, again. The single most common gap in the category applies here exactly as it does to steel. Backs are frequently bought in, and a titanium earring can arrive with a back that is not titanium at all.

Ask what was tested. Titanium containing no nickel is a compositional fact rather than a test result. That is fine, and it still leaves the other components and any surface treatment unaddressed.

If those four are answered, you have a titanium product you can reason about. If none of them are, you have a word.

Questions parents ask next

Is titanium worth the extra cost? If she has a confirmed nickel allergy, quite possibly. If she doesn't, you're paying to remove a variable that a tested steel has already measured as negligible.

Is anodised titanium safe? Anodising is a surface oxide effect rather than an applied coating, so it doesn't flake like plating. It does wear over time, which changes the surface.

What's Ti-6Al-4V? A titanium alloy containing aluminum and vanadium. Common, strong, and the ELI variant is the implant specification. Worth noting it's an alloy rather than pure titanium, so "titanium" alone still doesn't settle the question.

Is niobium another option? Yes, and it's genuinely inert and often used in body jewelry. It's less common, more expensive, and harder to find in children's designs.

She reacted to steel earrings once. Does that rule out S316L? Not necessarily. Most "steel" earrings aren't 316L, and the reaction may have been to a lower grade, to a plating, or to a back from a different supplier. A wear trial with a return policy behind it is the practical way to find out.
