Surgical steel vs stainless steel: what's the difference?

Surgical steel is a marketing term. Stainless steel is a family of alloys. The grade number is the only part that tells you how it behaves against skin.

Stainless steel is a family of hundreds of alloys. Surgical steel is a marketing term with no regulated definition in jewelry, usually meant to imply one of the implant-specification alloys within that family. Neither phrase tells you how a piece behaves against skin. Only the grade number does. Our Polished Surgical Steel collection states ours as S316L.

Surgical steel vs stainless steel, in short

Stainless steel is a technical category. Any steel alloy containing at least roughly 10.5 percent chromium qualifies, because that chromium forms a protective oxide layer on the surface. Hundreds of alloys meet that bar and they behave very differently.

Surgical steel is not a technical category in jewelry. It's a phrase, and no standard governs its use. When it means anything precise, it usually refers to 316L or a similar implant-specification alloy.

So they aren't opposites and they aren't the same thing. One is a family, the other is a claim about membership of the better part of that family, made without any requirement to prove it.

Why chromium matters

Everything about stainless steel starts with one property.

Chromium in the alloy reacts with oxygen to form a thin, invisible, self-repairing oxide film on the surface. That film is what stands between the metal and the world. Scratch it and it re-forms.

More chromium generally means a more robust film. Add molybdenum and the film resists chlorides far better, which matters because chlorides are present in perspiration and in swimming pools.

When that film breaks down, corrosion starts. Corrosion is what liberates metal ions into skin. So corrosion resistance and skin behavior are the same question asked two ways.

The grades that actually appear in jewelry

What the L means, and why it matters more than it sounds

The L in 316L stands for low carbon.

Carbon in steel tends to combine with chromium to form chromium carbides, and those form preferentially at grain boundaries. Where that happens, the local chromium available for the protective film is reduced, and those become the points where corrosion starts.

Lower carbon means fewer carbides, which means fewer weak points, which means the protective film holds more consistently across the surface.

For a kitchen sink that's a minor detail. For something sitting inside a piercing channel in a warm, damp, mildly acidic environment for years, it's the difference between a metal that stays inert and one that slowly doesn't.

The nickel question, properly answered

Austenitic grades, including both 316L and 304, contain nickel. Typically 10 to 14 percent. Nickel is what stabilizes the austenitic crystal structure, and that structure is what gives these grades their corrosion resistance and workability.

So a brand advertising nickel-free surgical steel is either using a specialist grade, which is rare and expensive, or hasn't checked.

The reason this is less alarming than it sounds comes down to a distinction almost no product page makes.

Presence is a question about composition. Release is a question about behavior. Only release affects skin.

Nickel held tightly inside a stable structure with an intact oxide film releases almost nothing measurable. Nickel in a corroding alloy releases considerably more. Same element, entirely different outcomes.

That's why a well-specified steel containing 12 percent nickel is frequently a better choice than a piece containing much less nickel in a less stable form.

EN 1811 is the test that measures it. An item sits in artificial sweat at body temperature for a week, and the solution is analyzed for how much nickel came out. It's a measurement rather than an adjective, and it's the only thing in this entire article that's an actual number about an actual product.

Where the phrase gets abused

Three patterns worth spotting on a listing.

"Surgical steel" with no grade. Could be 316L. Could be 304. Could be 430. If the seller can't tell you, they almost certainly don't know, because the information stopped somewhere up the supply chain.

"Surgical steel plated". That's a coating over something else, and the something else is undescribed. The coating wears through at the post first, where friction concentrates, and then the base metal is what's inside the piercing.

"Surgical steel" describing only part of the piece. Posts and backs are frequently bought in from a different supplier than the earrings themselves. A claim about an earring may not cover the components with the most skin contact.

That last one is the one that catches careful parents out, because there's no way to detect it from a photograph.

Why so many sellers genuinely don't know

This is rarely deception, and understanding why makes it easier to shop.

A retailer buys from a distributor who buys from a factory. At each step the description gets passed along as received. By the time a listing is written, the person writing it is several removes from anyone who could answer a metallurgical question, and "surgical steel" is the only description that survived the journey intact.

I've been through that chain. When I was sourcing, I asked the same specification questions at each supplier and got answers that didn't match each other. Some of what was described one way was clearly something else. There was no malice in most of it. Nobody in the chain was in a position to check.

That's why I turned down suppliers rather than accept a specification I couldn't verify. There's considerably more available on the market in styles and options than we carry. Taking it would have meant giving up the ability to answer the only question that matters.

Which one should she wear?

Reframe the question. It isn't surgical versus stainless, because those aren't two options. It's specified versus unspecified.

A named grade, ideally 316L, with the post and back confirmed as the same material, and a nickel release result from a named laboratory. That's a good choice for daily wear.

An unnamed "stainless steel" or "surgical steel" tells you nothing, which doesn't mean it's dangerous. It means you can't tell, and you'd be finding out on her ears over months of daily wear.

For a child that matters more than for an adult, because of timing. Nickel allergy affects around one in five people in North America, and the American Academy of Pediatrics reported in 2020 that nickel allergic contact dermatitis in children has nearly quadrupled over three decades, with at least 1.1 million children in the United States already sensitized. Sensitization is cumulative and generally permanent, and childhood is when the exposure starts.

How steel compares to the alternatives

Against low-karat gold. 9 karat is about a third gold and two thirds undisclosed alloy, commonly containing copper. A specified 316L is usually the better daily-wear choice.

Against sterling silver. 925 means 92.5 percent silver with the rest usually copper, which tarnishes and turns skin green. 316L is more stable.

Against implant-grade titanium. Roughly comparable. Titanium contains no nickel at all, and it's harder to work into fine settings, and "titanium" alone is as uninformative as "stainless steel" alone.

Against pure 24 karat gold. Different rather than worse. 24 karat has no alloy so nothing is present to react, and it's soft, so it works as a finish over a stronger core rather than as a whole earring.

Against plated anything. Better, because there's no coating to wear through and no unnamed base metal underneath it.

A quick test you can do at home

Not a grade test, and still useful.

Try a magnet. Austenitic grades like 316L and 304 are essentially non-magnetic. Ferritic grades like 430 are magnetic. A strong pull tells you it probably isn't one of the better grades. A weak or absent pull doesn't tell you which austenitic grade you have.

Look at the post rather than the front. On a plated piece the post is where the coating goes first. A color difference between post and front means you're already wearing the base metal.

Search the listing for a number. Not a word. A number. If there isn't one anywhere on the page, the page is describing appearance.

Two things this comparison should not be used for

It should not be used to write off steel jewelry generally. Most children wear inexpensive steel earrings with no difficulty at all, and a great deal of unspecified steel is perfectly serviceable. The argument here is about what you can know before you buy, not about what will definitely go wrong.

And it should not be used to assume a higher price bought a better grade. Price in jewelry tracks design, brand and precious metal content. A costly piece can be 304, and an inexpensive one can be 316L. The grade is stated or it is not, and price predicts neither.

The useful posture is neither trust nor suspicion. It is simply asking one question and noticing whether the seller can answer it.

Independently tested by SGS. I chose the pieces with the largest surface area touching an ear, and had the setting, the post and the butterfly back all tested, across eight pairs. That scope is unusual, and the reason is the one described above: backs commonly come from elsewhere, so many brands couldn't include them even if they wanted to.

Nickel release not detected under EN 1811. Tested to ASTM F2923 and CPSIA, with lead below a tenth of the United States legal limit. Report BTO 341803/1, and you can ask me for it.

Questions parents ask next

Is surgical steel stronger than stainless steel? That's not the axis. They aren't different metals. 316L is a particular stainless steel with better corrosion resistance than most, which is a different property from strength.

Does surgical steel rust? Well-specified austenitic grades resist it strongly. Cheaper ferritic grades can corrode with repeated exposure to moisture and chlorides.

Is "implant grade" the same as surgical steel? Closer to a real claim, because implant-grade specifications exist in medicine. In jewelry it still isn't regulated, so it still needs a grade behind it.

Can she swim in stainless steel earrings? 316L handles chlorides better than most grades because of the molybdenum. Drying the lobe properly afterwards matters more than the metal does.

Why do some steel earrings still irritate her? Either the grade is lower than advertised, the piece is plated over something else, the back came from a different supplier, or the irritation is mechanical rather than chemical.
