When ear piercings keep getting infected, it's often irritation rather than infection, and that distinction changes what you do about it. Irritation is skin responding to friction, pressure, trapped moisture or the metal itself, and it settles when the cause is removed. Infection is a clinical matter for your pediatrician. If it keeps coming back, the cause is usually something constant. Our safety promise sets out what we test and why.

This article can't diagnose anything

That needs saying at the top, because this is the one question where getting it wrong matters most.

See your pediatrician if there's spreading redness, the ear feels hot, there's swelling, the discharge is colored or thick, the pain is getting worse rather than better, she has a fever, a red streak is spreading from the ear, or a back has become embedded in the lobe.

Only someone who can examine the ear can tell you whether it's infected. Nothing below is a substitute for that, and this article won't tell you how to treat an infection.

What it can do is explain the things that irritate a piercing repeatedly, because those are within your control and they're frequently what's actually happening.

Irritation and infection get confused constantly

They can look similar early on, and they're different situations.

Irritation is skin responding to a physical or chemical cause. Friction, pressure, trapped moisture, or metal releasing into the channel. It's usually confined to where the contact is, it settles when the cause is removed, and it doesn't spread.

Infection involves bacteria multiplying in tissue. It tends to spread, brings heat, produces colored discharge, and gets worse rather than better with time. It needs medical assessment.

The reason this matters practically: repeated irritation gets treated as repeated infection, so the response is more cleaning and more antiseptic. That dries the skin, which irritates it further, which produces more of the symptoms that prompted the cleaning. The cycle continues while the actual cause sits untouched.

The back, pressed too far on

The most common cause and the easiest to fix.

A butterfly back should sit firmly, not tightly. Pushed hard onto the post it compresses the lobe and restricts blood flow, and compressed tissue is both irritated and slower to recover.

Check for an indentation when the earring comes out. There should be a small gap between back and ear.

Flat backs bring their own version: the disc creates a cavity that holds moisture against the lobe.

Movement and friction

A back that's too loose lets the earring shift constantly, abrading the lining of the channel. A tall setting catches on hair, collars and pillows and tugs the piercing.

Twisting or rotating an earring, which is still widely recommended, does the same thing deliberately.

Moisture that can't escape

The channel has almost no airflow. Water from swimming or showering sits in it, and skin kept damp is more easily irritated and less able to recover.

The metal

An earring releases metal ions into skin continuously. A metal that releases freely keeps the channel in a low-level state of irritation, permanently.

That's the one that produces the "keeps happening" pattern, because unlike a one-off knock, it never stops. The cause is present every hour the earring is in.

Nickel is the most common trigger by a wide margin. Nickel allergy affects around one in five people in North America, and the American Academy of Pediatrics reported in 2020 that nickel allergic contact dermatitis in children has nearly quadrupled over three decades, with at least 1.1 million children in the United States already sensitized. Copper releases readily too, and it's in most low-karat gold and in brass.

Stable metals such as S316L Surgical Steel release very little, which is why that grade is used for medical instruments and implants.

The pattern is the most useful thing you have

Trouble with certain earrings and not others points at the metal.

Trouble that started after a pair had been worn a while often means a coating has worn through at the post, exposing a base metal that was never described.

One ear worse than the other points at mechanics: sleeping side, or one back further on.

Trouble that has never settled since the day it was pierced is worth taking back to whoever performed it.

Trouble that follows swimming or hot weather points at moisture.

Write down which pair she was wearing each time. Two or three entries usually reveal what a year of guessing hasn't.

What to change

Take them out for a few days, if your doctor has confirmed there's no infection needing treatment. A settled piercing won't close in that time.

Check the back tension before anything else.

Stop twisting the earring.

Ease off the antiseptic. For a settled piercing, warm water and mild soap is usually enough, and frequent alcohol dries the skin.

Jewelry on last, off first. Shampoo, conditioner, sunscreen and hairspray all end up on the lobe.

Dry the ears properly after swimming and bathing.

Then look at what the earrings are actually made of, including the post and the back.

The setting, the post and the butterfly back all carry that same core. The post is the component inside the channel, and it's the one most commonly bought in from a different supplier and left out of any testing. I sent SGS the pieces with the largest surface area touching an ear and had all three components tested across eight pairs. Nickel release not detected under EN 1811. Tested to ASTM F2923 and CPSIA, with lead below a tenth of the United States legal limit. Report BTO 341803/1.

On the mechanics. The posts carry a double-notch system, so the butterfly seats at a set tension rather than gripping by friction. It holds without needing to be pushed hard onto the lobe, and it doesn't work loose and let the earring shift. Both failure modes at once.

We use butterfly backs rather than flat backs, which leaves the back of the lobe open rather than sealing a cavity against it.

Questions parents ask next

How do I tell irritation from infection at home? You can't reliably, which is why persistent or worsening symptoms go to your pediatrician. Irritation tends to stay put and settle when the cause is removed. Infection tends to spread and worsen.

Should she stop wearing earrings altogether? Usually not. If the pattern points at one particular pair or material, changing that is more useful than stopping.

Is it because her ears were pierced too young? Age of piercing isn't usually the driver of recurring irritation. What she has worn since matters more.

Could re-piercing fix it? Unlikely, if the cause is the metal or the back tension. Those follow her to a new piercing.

Does she need antibiotics every time? That's entirely your pediatrician's call, and it's one reason distinguishing irritation from infection matters. Repeated courses for something that isn't infection help nobody.
