Most reactions to earrings are not caused by the piercing, the cleaning routine, or anything you did. They are caused by metal being released from the earring into her skin. Different earrings release different amounts, which is why one pair is fine and the next pair is not. This guide explains what each symptom usually means and what actually reduces the cause. If an ear looks infected rather than irritated, that is a question for your pediatrician, and this guide is not a substitute for one. Our safety promise sets out what we test and why.

This is more common than most parents realize

Around one in five people in North America is affected by nickel allergy. The American Academy of Pediatrics reported in 2020 that nickel allergic contact dermatitis in children has nearly quadrupled over three decades, with at least 1.1 million children in the United States already sensitized.

Girls carry most of that burden, and the reason is exposure rather than biology. Girls are pierced earlier, wear earrings for longer, and change them more often. Across five European countries, being female raised the odds of nickel allergy more than fivefold. In one Norwegian population study, the single best predictor of nickel sensitivity was a reaction to inexpensive earrings.

The piercing itself matters too. A 2025 systematic review found people with piercings were nearly six times more likely to be nickel allergic, and estimated that roughly 82 percent of nickel allergy in the general population is attributable to piercing. Among Finnish children, those with pierced ears reacted to nickel at 31 percent, against 2 percent of those without.

None of that is a reason to panic, and none of it is a reason to feel responsible. It's a reason to know what the earrings are made of.

What "sensitive ears" actually means

It is worth being precise about the phrase, because it gets used for several different things and they need different answers.

Most of the time, sensitive ears means one of three situations. Her ears react to some metals and not others, which is a metal sensitivity and is the situation this guide is mostly about. Or her ears react to almost everything she puts in them, which usually means a metal she meets very often, and nickel is the most common candidate by a wide margin. Or her ears are physically delicate rather than chemically reactive, sore from pressure, friction or weight rather than from what the metal is releasing.

The three overlap and they are easy to confuse, but the fix is different for each. A metal sensitivity is solved by changing what the metal is. A pressure problem is solved by changing the design, the weight or how tightly the back sits. Cleaning routines help with neither.

What a reaction actually is

An earring sits in a warm, damp, slightly acidic environment for hours or days at a time. In those conditions, metals release ions into the skin around them. That happens with every metal to some degree. What varies enormously is how much.

Two different things can follow, and they are worth separating.

Irritation is a physical or chemical response. Friction, pressure, trapped moisture, or a metal releasing enough to bother the skin. It usually shows up fairly quickly, affects the area in direct contact, and settles once the cause is removed.

Allergic contact dermatitis is an immune response. The immune system has learned to recognize a specific metal, most often nickel, and reacts whenever it encounters it. It can appear hours after contact, can spread a little beyond the point of contact, and it comes back every time.

The difference that matters to you as a parent: irritation goes away when the cause does. An allergy doesn't. Once the immune system has learned a metal, that learning is generally permanent, and it has to be managed rather than cured.

That is the strongest argument for paying attention early. Reducing exposure is straightforward. Managing a lifelong sensitivity isn't.

When it starts after years of no problem

This is the version parents find most confusing, and it's completely normal.

Sensitization is cumulative. The immune system doesn't react on first contact. It reacts once it has been exposed enough times, over enough time, to learn the metal. That threshold varies from child to child, and it can be reached after months or after many years.

So a girl can wear earrings without any trouble from six to eleven, and then react. Nothing changed about her. What changed is that she reached the point where her immune system had seen enough.

There's a second reason it feels sudden, and it's worth understanding. Coatings delay the moment of contact. A plated or coated earring keeps the base metal away from skin until the coating wears through. By the time it does, and by the time a reaction follows, she has usually worn several different pairs. The pair that started it is long out of the picture, and it never gets identified.

That's why "her ears have always been fine" isn't the reassurance it sounds like, and why the material she wears every day through childhood is worth attention before anything goes wrong.

What each symptom usually means

Green or black staining on the skin

Green marks are a chemical reaction. Copper in the earring reacts with moisture and the acids in skin, forming copper salts that are green and that transfer onto the skin around the piercing.

Copper is present in most low-karat gold, in brass, and in many unnamed base metals. It's also what makes rose gold pink, which is why rose gold can't be made without it.

Black or dark gray marks are usually a different reaction, often involving abrasion between metal and skin, or a reaction with cosmetics, lotions and soaps. Both wash off. Neither is dangerous in itself. But both tell you the metal is releasing something.

Redness, itching and swelling

Redness around the piercing that appears within a day or two of putting a pair in, and settles when they come out, is the most recognizable pattern of a contact reaction.

Itching often comes first and it's easy to miss, because children rub rather than complain. Swelling of the lobe suggests a stronger response, and if it is significant, painful, spreading, or accompanied by heat or discharge, that moves out of this guide's territory and into your pediatrician's.

Crusting and build-up

A small amount of dried fluid around a piercing is ordinary, particularly on ears that are already irritated. Skin produces more fluid when it is inflamed, and it dries into a crust.

What matters is what comes with it. Crusting alone, on a settled piercing, is usually the skin responding to friction or to the metal. Crusting with heat, spreading redness, swelling, pain or colored discharge is a different situation and needs a doctor rather than an article.

Smell

Almost nobody discusses this and almost everybody encounters it. It's ordinary, and it isn't a sign of poor hygiene.

A piercing channel is a small enclosed space lined with skin, and it doesn't get much air. Skin sheds cells and produces oils continuously. In an enclosed, warm space with limited airflow, those cells and oils accumulate and are broken down by the bacteria that live on all skin. The result is a smell, and the reason it is often described as cheesy is that the process is genuinely similar to what happens in cheese ripening.

Two things make it worse: a back that traps material against the lobe, and a surface the debris can cling to. A smooth, well-finished, non-corroding post gives it less to hold on to.

Pain, especially at night

Pain in a settled piercing usually has a mechanical cause rather than a chemical one. A back pushed on too tightly compresses the lobe and restricts blood flow. A back that is too loose lets the earring move and drag. Sleeping on a stud presses it into the ear for hours.

Flat backs bring their own problem: the cavity behind the disc traps moisture against the skin, and skin held damp for long periods is more easily irritated and holds more build-up.

Why the same earrings behave differently on different children

Skin chemistry varies. How much a child perspires, the composition and pH of that perspiration, how much moisture sits against the metal: all of it changes how quickly a metal releases.

This is why one girl wears a pair for a year with no trouble and her sister reacts to the same pair in a week. Neither of them is doing anything wrong. The variable is the interaction between one particular skin and one particular metal.

It's also why "my older daughter was fine with these" isn't a reliable guide for the younger one.

What makes any of it worse

Five things, all of them ordinary.

Products applied over the earrings. Perfume, hairspray, lotion, sunscreen and soap all sit on the metal and against the skin. The rule that fixes it is simple: jewelry on last, off first. Everything else goes on before the earrings, and the earrings come out before anything else comes off.

Water left sitting. Swimming, bathing and showering aren't the problem in themselves. Moisture trapped between metal and skin afterwards is.

Sleeping in them, which turns a few hours of contact into a continuous eight.

A coating that has worn through, because the metal underneath was never the metal described on the label.

Backs pushed on too firmly. More pressure doesn't mean more secure. It means less blood flow.

What actually reduces the cause

Only one thing genuinely reduces the underlying cause, and it isn't a cleaning routine.

Reduce what the metal releases. That's a property of the material, and it's decided before you buy.

Two properties matter. Stability, meaning the metal resists corrosion and holds its constituents rather than shedding them. And being named, because a metal you cannot identify is a metal whose behavior you cannot predict.

This is measurable rather than a matter of opinion. The European standard EN 1811 exists specifically to measure nickel release during prolonged skin contact, by simulating a week of wear in artificial sweat and measuring what comes out. It measures release rather than presence, which is the distinction that matters, because nickel bound tightly inside a stable alloy behaves very differently from nickel in an unstable one.

Worth knowing: the United States has no legal limit on how much nickel an earring may release. The European Union introduced one in 1994 and tightened it in 2004. Denmark restricted nickel release in 1992, and nickel allergy among Danish children fell from 24.8 percent to 9.2 percent between 1985 and 1998. In the United States, that decision sits with you.

Where the line is, and where your doctor takes over

This guide covers what materials do to skin. It doesn't cover infection, and that distinction is worth holding clearly.

Contact your pediatrician if you see spreading redness, heat, significant swelling, colored or persistent discharge, pain that is getting worse rather than better, fever, or a back that has become embedded in the lobe. Those aren't questions about earring materials, and no article should be trying to answer them.

An earring can be the reason skin is irritated. Whether a particular ear is infected is a clinical judgment, and it needs someone who can look at it.

What to do this week if her ears are reacting

None of this is medical advice, and if the ear looks infected the first step is your pediatrician rather than any of the below. But if her ears are simply sore, itchy or marked, there is a sensible order to work through.

Take the earrings out and give it a few days. This sounds obvious and it is the step most often skipped, usually because a parent is worried the hole will close. A settled piercing that has been in place for months does not close in three days. If her ears calm down quickly, that tells you something useful on its own: the cause was the earring rather than the piercing.

Keep a note of which pair. Write down which earrings she had in when it started. Parents rarely do this, and it is the single reason so few reactions ever get traced back to a specific pair. Two or three entries is usually enough to see a pattern.

Change one thing at a time. If you swap the earrings, the cleaning routine and the sleeping arrangement all in the same week, you will not know which one helped. Change the earrings first, because they are the most likely cause.

Check what is going on the ears, not just in them. Shampoo, conditioner, sunscreen and perfume all end up on the lobe. Jewelry on last and off first is a small change that solves a surprising number of cases on its own.

Look at the back, not just the front. Feel whether it has been pushed on tight enough to indent the lobe. Check whether the back is a different color or finish from the front, which suggests it came from somewhere else and may be a different metal entirely.

Then look at what the next pair is actually made of. Not the word on the label, but the named metal and grade, all the way through to the post and the back.

If a pattern is emerging and you want it confirmed properly, a doctor can arrange patch testing, which identifies exactly which metals she reacts to. That is worth doing if reactions keep happening, because knowing the specific metal turns guesswork into a shopping rule she can use for the rest of her life.

Nickel release was not detected under EN 1811. Tested to ASTM F2923 and CPSIA, with lead below a tenth of the United States legal limit. That is report BTO 341803/1, and you can ask me for it.

There is one more reason we hold the standard this far down. An earring is not a medical device. But she is pierced at five or six and wears metal in her body more or less continuously into adulthood, which is closer to an implant than to anything else she puts on. That length of contact is why the grade is worth arguing about.

If her ears have already reacted, the sensible thing is not to take my word for any of this. Buy one pair, let her wear them, and send them back inside 30 days if they do not work for her. Skepticism after a bad experience is reasonable, and it should not have to cost you anything to test.

Questions parents ask next

Can a reaction be reversed? Irritation settles once its cause is removed. An allergy, once established, is generally permanent and is managed by avoiding the metal rather than by treating the skin.

Does she need to stop wearing earrings altogether? Usually not. The question is which metal, not whether she can wear them at all. Many children who react to inexpensive earrings wear stable, named materials without trouble.

Is it the piercing or the earring? A settled piercing that reacts to some earrings and not others is telling you it is the earring. A piercing that has never settled is worth showing to whoever performed it.

Are more expensive earrings safer? No. Price tracks design, brand and gold content. A costly 9 karat piece can release more than an inexpensive well-specified steel one. The specification predicts behavior. The price doesn't.

How long before I know if a pair suits her? Contact reactions usually show within a few days of consistent wear. Cumulative sensitization is a much longer process, which is why the everyday material matters more than any single pair.

Her ears have never reacted. Does any of this apply? It applies most to her. No reaction now is not evidence of no reaction later, and childhood is when the exposure that leads to sensitization begins.
