What does "surgical steel" actually mean?

"Surgical steel" is not a regulated standard. Here is what the phrase does and does not tell you, and the two questions that replace it.

Almost nothing, on its own. "Surgical steel" isn't a regulated term, there's no specification behind it, and no authority checks who uses it. It's a description a seller chooses, not a standard a product passes. The phrase usually points at the 300 series of stainless steels, which is a family rather than a single alloy, and the family includes grades with very different amounts of nickel available to skin. The information you actually need is the grade, and our materials page names ours.

That's an uncomfortable thing for us to publish, because our own steel range gets called surgical steel too. It's still true, and you're better off knowing it.

What the phrase is pointing at

A family, not a metal. Stainless steel isn't one substance. It's hundreds of alloys of iron with chromium and usually nickel, and the recipe changes what the metal does. The 300 series is the group most jewelry comes from.

Two grades do most of the work. 304 is the common one, roughly 18% chromium and 8% nickel. 316L is the one with molybdenum added and the carbon reduced, which improves how it resists corrosion. Both get sold as surgical steel. They aren't the same thing against skin.

The word "surgical" is doing the persuading. It suggests an operating theater, and that association is the whole reason the phrase sells. Real surgical instruments are made from steel, so the link isn't invented. It just doesn't transfer to an earring the way a shopper assumes it does.

Nobody polices it. There's no body that certifies a product as surgical steel, no test it passes, and no penalty for using the phrase loosely. Blue Nile, which is about as mainstream as jewelry retail gets, puts it bluntly on its own site: the term has no formal definition.

Why this matters for a child

Nickel is the reason. It's the most common contact allergen in the developed world and it's present in most stainless steel, including the grades sold as surgical steel. That isn't a scandal on its own, because nickel locked inside a stable alloy behaves very differently from nickel that migrates out onto skin.

Corrosion is the mechanism. Nickel reaches skin when the alloy corrodes. A pierced ear is warm, slightly damp and mildly acidic, which is a more demanding environment than a kitchen sink. A grade that resists corrosion well holds its nickel. A grade that doesn't, releases.

So the grade is the whole question. Two earrings can both be honestly described as surgical steel and behave completely differently on the same child, because one is 316L and the other is 304, or because one is 316L and the other is something a supplier called 316L.

Which is why the label can't help you. It doesn't distinguish between those cases. It was never designed to.

The standards that do mean something

There are real specifications. They're just not the ones printed on the packet.

ASTM F138 is the specification for stainless steel used in surgical implants. It calls for 316LVM, which is 316L that's been vacuum melted to reduce inclusions. This is the steel standard, and a product either meets it or it doesn't.

ASTM F136 is the equivalent for titanium, covering Ti-6Al-4V ELI. When the piercing community says "implant grade titanium", this is the specification it means. The Association of Professional Piercers points people at it for fresh piercings.

ASTM F67 covers commercially pure titanium for the same use.

EN 1811 is different in kind, and for a parent it's the most useful of the lot. It isn't about what the metal is, it's about what comes off it. A sample sits in artificial sweat for a week and the nickel released is measured. It gives you a number in micrograms per square centimeter per week, and the limit for an item in prolonged skin contact is 0.5.

ASTM F2923 is the American children's jewelry safety specification. It covers lead, cadmium, nickel and mechanical hazards for jewelry intended for children twelve and under.

Notice what separates these from "surgical steel". Each one has a number, a document, and a test that either passes or fails.

The honest complication

There's a criticism of steel jewelry that deserves a straight answer rather than a defensive one.

The piercing community's position is that for a fresh piercing, implant grade titanium to ASTM F136 is the right choice and steel is second best. They're correct, and they've earned that position by watching what happens to healing tissue. A fresh piercing is an open wound. The bar for what sits inside an open wound should be higher than the bar for a healed lobe.

That community also warns, rightly, that "implant grade" gets abused just as freely as "surgical steel". One of the most upvoted threads on the subject is a piercer pointing out that most titanium sold as implant grade has no documentation behind it.

Here's where we sit in that argument. purepixxi earrings are for fully healed ears, not for piercing and not for healing. If you're choosing jewelry for a fresh piercing, talk to a reputable piercer and ask for ASTM F136 titanium. That's not a sales position, it's the right answer to a different question.

For a healed ear worn every day, the question changes shape. The tissue is closed. What matters is how much nickel reaches the skin over months of contact, which is exactly what EN 1811 measures.

What to ask instead

Two questions replace the phrase entirely.

"What grade is it?" You want an alloy designation, not an adjective. 316L. 304. Ti-6Al-4V ELI. A seller who knows will tell you immediately. A seller who doesn't will repeat the marketing word back at you, and that answer is itself informative.

"What did the nickel release test measure?" This is the one almost nobody asks and the one that settles it. You're asking for an EN 1811 result with a number. "Nickel free" isn't a test result. "Hypoallergenic" isn't a test result. A figure in micrograms per square centimeter per week is.

A third, if you want to be thorough. Ask what the post and the back are made of, not just the front. Plating wears first at the post and the butterfly, where friction concentrates, and the front can still look perfect while the base metal underneath is in contact with the piercing channel.

What we do

We write S316L Surgical Steel rather than surgical steel, every time, on every product page and in every email. It looks fussy. It's deliberate, because the grade is the part that carries information and the adjective is the part that doesn't.

Our Polished Surgical Steel range is S316L throughout. Not a coating over something cheaper, not a core with a different post screwed into it. One material, front, post and butterfly back.

Our gold range is 24K Pure Gold applied onto a solid S316L Surgical Steel core with Aunix™ 24K, our engineered gold finish. It's a gold-plating process and we say so, because a plating that's described as something else is exactly the kind of claim this article is about.

Both are independently tested by SGS. Nickel release under EN 1811: not detected. Tested to ASTM F2923 and CPSIA, with lead well below the United States legal limit. Report BTO 341803/1, and you can write and ask me for a copy.

One thing we don't claim. Our steel isn't certified to ASTM F138. That standard is for surgical implants, we don't make implants, and we're not going to borrow the credibility of a specification we haven't tested against. What we have is the measurement that applies to earrings on healed ears, and we publish it.

Questions parents ask next

Is surgical steel the same as stainless steel? Surgical steel is stainless steel. The word surgical adds an impression rather than a property. Stainless steel is the accurate description and the grade is the useful detail.

Does surgical steel contain nickel? Almost always, yes. Both 304 and 316L contain nickel. The question that matters isn't whether nickel is in the alloy, it's how much escapes onto skin, which is what EN 1811 measures.

Is 316L better than 304 for sensitive ears? It resists corrosion better, which is the mechanism by which nickel reaches skin, so it's the more sensible choice for something worn daily. That's a reason to prefer it, not a guarantee about any individual child.

If the label says surgical steel, is it a lie? Usually not. It's vague rather than false. The seller may well be describing 316L accurately and simply using the common term for it. You just can't tell from the label which case you're in.

What if a brand won't tell me the grade? Treat that as your answer. A brand that knows its own specification has no reason to withhold it, and one that can't tell you probably doesn't know what its supplier sent.
